Hospice Renfrew is pleased to offer Heart at Home, a program that offers compassionate grief and bereavement support to individuals in the comfort of their own homes. Hospice Renfrew’s trained volunteers will help ensure that those experiencing loss will feel supported, connected, and cared for through a listening ear, and meaningful conversation. 

The first step in applying to the Heart at Home Program is to Fill Out The Referral Form         Download and save the form to your computer before you fill it out.
